Estratégias de dosagem do HIIT e sua influência no vo2máx em pacientes com insuficiência cardíaca: uma revisão sistemática

Introdução: O treinamento intervalado de alta intensidade (HIIT) é uma estratégia eficaz na reabilitação cardíaca, melhorando significativamente a capacidade aeróbica. No entanto, a falta de clareza quanto à dosagem ideal limita a sua aplicação clínica. Objetivos: Avaliar e determinar a dosagem ideal de treinamento intervalado de alta intensidade (HIIT) para melhorar o Vo2máx em pacientes com insuficiência cardíaca. Métodos: Foi realizada uma busca em 5 bases de dados (PubMed, Scopus, Web of Science, Cochrane Library e Embase). Foram incluídos estudos que utilizaram o protocolo HIIT e mediram o Vo2máx. E compararam o HIIT com treinamento de intensidade moderada (MICT), intervalos curtos de intensidade máxima (SID) ou algum programa de força em pacientes com IC. Resultados: foram analisados 7 estudos que incluíram um total de 625 participantes com fração de ejeção do ventrículo esquerdo (FEVE) entre 35% e 50%. Os resultados indicam que o HIIT é mais eficaz na melhoria do VO2máx do que outros métodos. O HIIT oferece flexibilidade em intensidade, frequência, duração e intervalos de trabalho. Recomenda-se um mínimo de 3 sessões semanais durante pelo menos 6 semanas, com intensidade superior a 85%, recuperação ativa a 40-50% do VO2máx, intervalos de trabalho de 30 segundos seguidos de recuperação, 4-7 séries por sessão e um tempo total mínimo de 25 minutos por sessão. Estas diretrizes podem otimizar as adaptações fisiológicas ao treinamento em pacientes com insuficiência cardíaca. Conclusão: Os resultados da revisão sistemática apoiam a eficácia do HIIT como opção de treino físico para pacientes com IC.
