Revisión sistemática sobre la optimización de la actividad física para el VIH y condiciones crónicas

Introducción: La actividad física es esencial para la promoción de la salud, especialmente en personas con enfermedades crónicas como el VIH/SIDA, las hepatitis B y C, la EPOC y la artritis reumatoide. Esta revisión sistemática busca sintetizar la investigación existente sobre los beneficios, los riesgos, los tipos recomendados, las directrices y la dosificación de la actividad física para personas con estas enfermedades crónicas.
Método: Siguiendo las directrices PRISMA, se incluyeron ensayos controlados aleatorizados, estudios de cohorte, estudios longitudinales y estudios transversales publicados en revistas científicas en inglés con revisión por pares. Las fuentes de datos incluyeron PubMed, Google Scholar, Scopus, Web of Science y PsycINFO. Los términos de búsqueda se centraron en la actividad física y las enfermedades crónicas, utilizando operadores booleanos para una cobertura completa. El riesgo de sesgo se evaluó mediante la herramienta de evaluación crítica del JBI, y la solidez de la evidencia se evaluó mediante el marco GRADE.
Resultados: La actividad física reduce significativamente el riesgo de enfermedad cardiometabólica, mejora la fuerza, la salud cardiovascular y la función inmunitaria en personas con enfermedades crónicas. Los regímenes eficaces incluyen entrenamiento aeróbico y de resistencia combinados, ejercicios cardiorrespiratorios y programas domiciliarios. Sin embargo, los ejercicios de resistencia pueden aumentar los marcadores inflamatorios y el estrés oxidativo, lo que requiere programas personalizados y supervisión médica para garantizar la seguridad.
Conclusión: La actividad física ofrece importantes beneficios para la salud de las personas con enfermedades crónicas como el VIH/sida, las hepatitis B y C, la EPOC y la artritis reumatoide. Esta revisión proporciona directrices basadas en la evidencia para que los profesionales de la salud optimicen las intervenciones de actividad física, haciendo hincapié en las prescripciones individualizadas para maximizar los beneficios y minimizar los riesgos.
